At North Country Health Consortium, we believe everyone deserves the opportunity to live a healthy, connected, and supported life.
We are a nonprofit organization working across northern New Hampshire to improve health and strengthen communities. But our work doesn't happen in one place, through one program, or with one solution.
Every day, we work alongside individuals, families, schools, healthcare providers, community organizations, and partners across the region to address the many factors that influence health and well-being.
That can mean helping a new parent access support during pregnancy and postpartum. Connecting someone with recovery resources. Creating opportunities for young people to lead, learn, and thrive. Helping build and sustain the healthcare workforce our rural communities depend on. Or bringing organizations together to tackle challenges that no one organization can solve alone.

Since 1997, NCHC has served as a convener, connector, and partner in northern New Hampshire. We listen to what communities need, bring the right people and resources together, and work collaboratively to create solutions that are responsive to the unique strengths and challenges of our rural region.
Our programs may look different from one another, but they share a common purpose: to improve health, strengthen connections, and help northern New Hampshire communities thrive.
How We Make an Impact
Our work reaches across communities and across the lifespan. We:
- Build Healthy Communities by bringing partners together to understand local needs, strengthen systems, and improve community health.
- Support People & Families by connecting individuals with resources, education, and services that support health and well-being.
- Strengthen the Healthcare Workforce by educating, training, and supporting the professionals our rural communities depend on.
- Prevent Substance Misuse & Support Recovery through prevention, education, harm reduction, connection, and recovery-focused initiatives.
- Invest in Youth by creating opportunities for leadership, prevention, education, and career exploration.
