Ways2Wellness CONNECT is a program that provides tools and resources to positively impact the health and well-being of residents of the Coos, Carroll, and Northern Grafton Counties of NH, including Community Health Workers (CHWs) who can help you or your patient, ages 18 and over, to overcome the barriers to achieving overall health and wellness.
