Connect

The North Country Health Consortium has partnered with NAMI NH to expand the Connect Project to the Lancaster and Colebrook areas. Connect is a National Best Practice Program developed by the National Alliance on Mental Illness, NH (NAMI NH) that provides suicide prevention training and support services to communities and key service providers in NH and several other states. The project focuses on how to reduce youth suicide by:

  • Recognizing people at risk for suicide
  • Connecting with those at risk
  • Connecting the person at risk with appropriate services to assure his/her safety
  • Connecting the community around working together to prevent suicide

Trainings in prevention and postvention are available for schools, police, social services, mental health, EMS, faith leaders, hospitals, and community members.

Chronic Disease Self-Management (CDSM)

The CDSM program is a lay-led participant education program offered statewide. Participants are adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease, and diabetes; their family members, friends and caregivers can also participate. The program is 2.5 hours per week over a 6-week period, and provides information and teaches practical skills on managing chronic health problems.
